Reduced SSN API footprint (backend)

• Remove SSN entirely from general license/provider API
• Provide only masked SSN (last four) via API to staff users with access to practitioners’ private data

NOTE
We will need to discuss how we want to handle rolling out this breaking API change

@jusdino We have word that the EDs are ready to move forward with the revoke privilege feature. Putting this back in the backlog for now.
