Proxy phone calls

This is likely incredibly difficult and community help would be appreciated.

If I'm a Matrix user and start a call with a phone number (possibly multiple), that should proxy somehow to a real phone call through Twilio. Likewise, if a real phone calls my Twilio number then I should be able to answer it.
