Command 'Continue: focus continue chat' should not be user visible or should be more clear

To reproduce

1. Install extension
2. Trying to figure out how to see any 'Continue' UI components. I'm trying to find the chat interface.
3. Repeatedly try to run the command 'Continue: focus continue chat'
4. Each time, a UI component asks me to 'Enter the session ID'

For reference, you should be able to open the chat UI using this menu but it doesn't seem to be working either 🤔
